Fresh Start Request Form

Thank you for completing the online application for the Fresh Start Back-to-School Program. 

If we have any questions, we will call or email you. Please respond promptly to any requests for additional information.

You will receive a confirmation email from Falmouth Service Center if your child is enrolled. The program pick-up day is August 18th from 11:00 am to 5:00 pm. If you have any questions about your child's eligibility for the program, please call us at (508) 548-2794.

If you are in need of food and you don't live near FSC, please visit the Greater Boston Food Bank website for a list of regional food pantries in your area.
