Fresh Start Request Form

Thank you for completing the online application for the Fresh Start Back-to-School Program. 

The Falmouth Service Center will send a confirmation email once your application is approved. Please check your email within one week of submitting your application.

If we have any questions, we will call or email you. Please respond promptly to any requests for additional information. If you have any questions about your child's eligibility for the program, please call us at (508) 548-2794.

If you are in need of food and you don't live near FSC, please visit the Greater Boston Food Bank website for a list of regional food pantries in your area.
